A formal investigation has been launched into the death of a passenger who fell onto the tracks and was fatally struck by a train at Ickenham station in west London last month.
The tragic incident occurred at approximately 10:30 pm on March 28. Ickenham station, which is served by both the Piccadilly and Metropolitan lines, was the scene of the fatality after the passenger fell from the platform and was hit by an oncoming train.
The individual sustained fatal injuries and was later discovered by London Underground station staff.
Today (April 16), the Rail Accident Investigation Branch (RAIB) confirmed it had launched an inquiry into the circumstances surrounding the incident. The investigation will focus on the sequence of events leading up to the fall, as well as examining the actions of those involved, the management of station staff, and existing safety protocols in place to prevent such accidents.
The RAIB will also explore any underlying systemic or management issues that may have contributed.
TfL Responds
Lilli Matson, Chief Health, Safety and Environment Officer at Transport for London (TfL), offered condolences and assured the public that a full internal review is also underway.
“Our thoughts are with the family and friends of the person who sadly died at Ickenham Tube station last month.
“We are supporting the RAIB with its investigation and are conducting our own internal inquiry to understand the causes and any actions that can prevent such tragic incidents in future.
“We undertake a huge range of activity to ensure our network is safe for everyone at all times, and are constantly working to eradicate the risk of such incidents.”
Ickenham station has since reopened and continues to operate normally, while both investigations remain ongoing.
