An NHS trust has launched an independent patient safety investigation into the care of William Chapman, the 24-year-old man who was fatally mauled by a tiger at Yorkshire Wildlife Park.

Rotherham Doncaster and South Humber NHS Foundation Trust (RDaSH) confirmed the investigation following the death of Mr Chapman, whose family had previously raised concerns about his contact with mental health services in Doncaster.

The investigation was confirmed at an inquest opening in Doncaster on Thursday.

The hearing was told that Mr Chapman had experienced mental health difficulties for a significant period and had been diagnosed with schizophrenia, with several episodes of psychosis.

However, the inquest also heard that he had been considered “highly functioning and independent with no care requirements” while receiving support from adult mental health services in Doncaster.

Family concerns being examined

Coroner’s officer Caitlin Brookes said Mr Chapman’s family had raised “a number of concerns” about his contact with mental health services.

RDaSH said it had commissioned an independent patient safety incident investigation following the fatal incident, with the agreement of other agencies.

A trust spokesperson said: “Our deepest sympathies are with the family and friends of William Chapman.

“Following the incident, RDaSH has commissioned an independent patient safety incident investigation with agreement from other agencies.

“This remains underway.”

The trust said Mr Chapman’s family was aware of the investigation and would be fully supported to participate in the process.

RDaSH said it would not comment further on his care while the investigation remains ongoing.

Inquest adjourned

Mr Chapman was a self-employed online trader who was born in Chichester, West Sussex, and was living in Doncaster when he died.

The inquest was opened by Senior Coroner Nicola Mundy on Thursday before being adjourned until March 18, 2027.

The circumstances surrounding the tiger attack and the events leading up to Mr Chapman entering the animal enclosure are continuing to be examined separately.

The NHS investigation does not establish that any failing in Mr Chapman’s care contributed to his death; it is intended to examine the care provided and identify any patient-safety issues.
