Forklift Tragedy: Mechanic Killed in Workplace Accident
Joseph Robinson, 39, a mechanic from Clitheroe, died after a forklift accident at E. Jackson (Chatburn) Limited’s site in Clitheroe on 13 October 2020. While working with a new employee to fix a steering fault, Joseph instructed the driver to move the forklift forward. Instead, the vehicle reversed, trapping him against a trailer.
Severe Injuries Lead to Tragic Death
Joseph was rushed to hospital with severe head injuries and placed in an induced coma. He died the next day. On 15 October, his organs were donated, helping eight people survive thanks to his generosity.
“Joe was a much-loved son, brother, nephew, cousin and friend. He was a hero in our eyes,” said his mother, Sue Robinson. “He loved taking things apart as a boy—he was always meant to be a mechanic. Losing him has robbed us of so much.”

Safety Failures Blamed for Fatal Accident
The Health and Safety Executive (HSE) investigation revealed serious failures at the firm. The forklift was over 30 years old, badly maintained, and hadn’t undergone mandatory LOLER inspections for years. No safe working procedures were in place — no designated repair zones or clear separation between pedestrians and vehicles.
HSE inspector Shellie Bee blasted the company: “This incident could so easily have been avoided. The employer allowed an untrained worker to operate a dangerous vehicle in unsafe conditions. Employers must assess risks and follow strict safety rules.”
Company Fined £30,000 for Safety Breaches
E. Jackson (Chatburn) Limited pleaded guilty to breaching the Health and Safety at Work Act 1974. At Crewe Magistrates’ Court in April 2023, the company was fined £30,000 and ordered to pay costs of £3,712.80.
