Engine Failure Sparks Emergency Touchdown
Disaster struck on the return leg from a training flight near the Humber Bridge when the number 3 cylinder and piston violently broke free from the plane’s engine. The sudden engine failure forced the pilot to make a dramatic forced landing in a nearby field. Thankfully, while the aircraft suffered serious damage, the passenger onboard escaped with only minor injuries.
Fatigue Cracks Blamed for Engine Breakdown
Investigations revealed a nasty fatigue crack had caused the cylinder studs to fail. These critical studs had been replaced during an overhaul, but problems arose when several stud threads stripped before the bolts could be properly tightened. This wasn’t an isolated case—similar thread-stripping issues popped up in other engines of the same model.
Faulty Fasteners Under Fire
Experts dug deeper and found that the nuts fastening the cylinders were the real culprits. They distorted under pressure, damaging the stud threads before they could hit the correct torque settings. In some cases, the nuts failed just after reaching the required torque, risking further mechanical failure.
