Probe launched after tragic death at Ickenham Tube station
A formal investigation is underway after a passenger fell onto the tracks and was fatally hit by a train at Ickenham station in west London last month.
Fatal fall at Ickenham station
The horrific incident happened around 10:30pm on March 28. The station, served by both the Piccadilly and Metropolitan lines, saw a passenger fall from the platform and suffer fatal injuries after being struck by an oncoming train.
London Underground staff later discovered the victim.
RAIB launches inquiry into incident
On April 16, the Rail Accident Investigation Branch (RAIB) confirmed it had opened a formal probe. They will examine the exact sequence of events leading to the fall, station staff actions, safety protocols, and whether any systemic management issues played a role.
TfL issues statement and safety pledge
Lilli Matson, Chief Health, Safety and Environment Officer at Transport for London (TfL), expressed condolences and vowed a full internal review.
“Our thoughts are with the family and friends of the person who sadly died at Ickenham Tube station last month.
“We are supporting the RAIB’s investigation and conducting our own inquiry to understand causes and how to prevent such tragic incidents in future. We continually work to keep our network safe and reduce risks.”
Station reopens amid ongoing investigations
Ickenham station has since reopened and remains fully operational. Both investigations by TfL and RAIB are ongoing.
