The Independent Office for Police Conduct (IOPC) is continuing its investigation into the actions of Avon and Somerset Police officers following the tragic death of 17-year-old Tamzin Hall on the M5 earlier this month.

Incident Details

Tamzin Hall was being transported to a custody suite in Bridgwater on November 11 after her arrest during a disturbance in Taunton at around 10:40 PM. She was handcuffed in the rear seat of a marked police Toyota Corolla, with an officer seated beside her.

According to initial accounts from the police driver, the car pulled onto the hard shoulder of the northbound carriageway near junctions 25 and 24 at around 11 PM for safety reasons. During this stop, Tamzin exited the vehicle and was struck by a passing vehicle on the southbound carriageway. Despite the swift arrival of paramedics, she sadly died at the scene.

Investigation Update

The IOPC has made significant progress, collecting witness accounts, and reviewing CCTV and body-worn camera footage. Both officers involved have been issued misconduct notices, signalling potential breaches of duty and responsibility.

An IOPC spokesperson emphasized that these notices do not imply guilt and are a procedural step in the investigation. Whether disciplinary proceedings will follow remains under review.

Statement from the IOPC

David Ford, IOPC Regional Director, stated:
“My thoughts and sympathies remain with Tamzin’s family and friends, and everyone affected by the tragic events of that evening.

He added, “We are working hard to establish the exact circumstances from Tamzin’s arrest to the moments leading to her death on the M5. We will ensure her family receives regular updates as the investigation progresses.”

The focus of the Investigation

The inquiry aims to examine the officers’ actions, decision-making, and risk assessments before Tamzin’s death. Investigators are assessing whether their conduct adhered to relevant policies and training.

Inquest Opened

An inquest into Tamzin’s death has been opened and adjourned, pending the outcome of the ongoing investigation.

Support for the Family

The IOPC has met with Tamzin’s family to offer condolences and explain the investigation process. They will be kept informed as new details emerge.

Community Impact

Tamzin’s death has prompted widespread sorrow, with calls for a thorough investigation into police protocols and decision-making. The IOPC continues to work to provide answers to her grieving family and the public.
